BAPTISM REGISTRATION

Baptism Registration (For children younger than 7 years of age)

Godparent Baptismal Form

Policy: Parents must be a registered and active (use of envelopes or on-line giving) parishioner of St. Frances Cabrini or one of our Quo Vadis Sister Parishes for 3 months before a date can be reserved. Once you submit your request for a baptism, please email child’s birth certificate and the signed/filled godparent forms to mramos@qvdomine.org in order to start the baptism process.

Policy: Bride or groom must be a registered and active parishioner for 6 months, or the child of one, before a date can be reserved. If eligible to reserve a date: please note, this is only a request form. Confirmed date/time will occur once approved by the priest. Do not make any plans, until you receive confirmation.
